SUBJECT: Compensation Forms in Portable Document Format (PDF)

1. PURPOSE

1.1 The purpose of this directive is to provide users (departments, agencies, crowns and pay offices) with information on access and use of fillable Compensation Forms in Portable Document Format (PDF).

1.2 In this text, use of the masculine is generic and applies to both men and women.

2. BACKGROUND

2.1 In 1999, electronic compensation forms were made available on the Compensation Internet and Publiservice sites. We are progressively making all compensation forms available in fillable PDF.

3. PROCEDURES/INSTRUCTIONS

3.1 These forms may be viewed, filled-in on the screen and then printed. To view and print the forms, the user will require access to Acrobat® Reader version 4.0 or more recent versions. This Acrobat® Reader version 4.0 is a software available free of charge and can be downloaded from the Adobe Web site hereunder. Adobe Reader

Note: Earlier versions of Acrobat® Reader will not work properly and may cause printing problems.

3.2 To open a fillable form in PDF format, the user needs to click on either the form title or the form number (see the example below). Once the correct form has been selected, the user may complete the form on their computer screen and then print it. Please note that filled and completed forms cannot be saved.
