Today, the form we're covering is called Certification of Lock-In for Purposes of the Public Service Superannuation Act or the Pension Benefits Division Act PWGSC-TPSGC 2347-18.
This form confirms that you will move or deposit funds from your public service pension plan to a locked-in registered retirement account of your choice.
The reason why funds need to be "locked-in" is to provide you with retirement income.
The form is necessary for the payment of a transfer value, a pension benefit division, or an excess amount from the lifetime retirement benefit portion of a pension transfer agreement.

You must complete it with your financial institution. Before we move on, please note that this video will show you how to complete the form for a Transfer Value Payment.

There are two ways you can complete the form, electronically or print a blank version and fill it out by hand with a dark ink pen.
Enter your personal information in the appropriate fields.
Remember that you must enter either your Personal Record Identifier, (your PRI) and/or your Pension Number.
We need one of these to locate your file.
(The form CERTIFICATION OF LOCK-IN FOR PURPOSES OF THE PUBLIC SERVICE SUPERANNUATION ACT OR THE PENSION BENEFIT DIVISION ACT appears full screen. A male voice explains how to fill out the form. The Plan Member’s Personal Information section appears. When the voice says “… remember that you must enter either your Personal Record Identifier …,” the PRI-CIDP field becomes highlighted in yellow. When the voice says “… and/or your Pension Number,” the Pension Number—numero de pension de pension field becomes highlighted in yellow.)

You should take the time to read the instructions carefully in addition to watching this video. Let's go through the fields or boxes.
First, add your full name in the Name of Payee box.
Next, enter your Social Insurance Number.
In the Account Number box, indicate the locked-in account number to which the funds will be transferred.
Please note that it is crucial that the account number entered corresponds to the account number indicated on the Canada Revenue Agency direct transfer of funds form T2151.
Your financial institution must complete the bottom part of this page.
They must also indicate their complete name and address.
Please note that this is the address where the payment will be mailed.
It may differ from the physical address of your financial institution. Next, an official Company or Financial Institution Stamp, Seal or Address Stamp is needed. This is a mandatory requirement.
The remaining fields identify the financial institution official completing the form. They need to indicate their name, position, title, telephone and fax numbers.
They must then sign with the date they completed the form.

Once completed, mail the form to the Pension Centre. Please include your completed Direct Transfer of Funds form (T2151).
