Proof of death of a member of the public service pension plan
In the event of a member/former plan member's death, the Government of Canada pension centre should be contacted immediately, and the death certificate issued by the funeral director showing the date of death should be sent along with the required supporting documentation that is listed in the following sections:
Situations where proof of death is required
- Plan member or former plan members with survivors (widow/er, partner, and/or children under age 25)
- Survivor (widow/er, partner or child under age 25)
- Plan member of former plan member with no survivors

Plan member or former plan member with survivors (widow/er, partner, and/or children under age 25)
Required Information
- Name of deceased
- If pensioner is receiving survivor benefits, provide the second superannuation number
- Name of person calling/writing
- Relationship to deceased
- Survivor's Social Insurance Number
- Telephone no. for survivor
- Language preference of survivor
- Proof of death
- Confirmation of children under the age of 25 and proof of age
- Letter from survivor to request (if applicable):
- Direct deposit
- Public Service Health Care Plan (PSHCP)
- Continued Dental Plan coverage
- Return of all cheques issued after the month of death
The survivor's superannuation pension number will be the same as the annuitants and must be used in all communications with the Government of Canada pension centre.
Survivor (widow/er, partner or child under age 25)
Required Information
- Name of deceased
- Superannuation pension number
- Name of person calling/writing
- Name, address and telephone number of Executor of estate
- Language preference
- Proof of death
- Return of all cheques issued after the month of death
- If the survivor was also an annuitant, provide the second superannuation number
Pensioner with no survivors
Required Information
- Name of deceased
- Superannuation pension number
- Name of person calling/writing
- Name, address and telephone number of Executor of estate
- Language preference
- Proof of death
- Written confirmation that there are no eligible survivors "legal spouse, common-law spouse, same-sex partner, children under the age of 18 and students between 18-25 years of age"
- Return of all cheques issued after the month of death
